Home Care Package Inclusions and Exclusions
Your eligibility for a particular level is determined by an assessment conducted by an Aged Care Assessment Team (ACAT). Once your care package is approved, we work with you to create a tailored support plan that maximizes the funding and meets your specific needs.
Services provided under a Home Care Package are based on an individual’s assessed care needs, agreed care plan and individual package budget.
